By checking the box below, I hereby sign and give permission for the camper listed above to participate in the NGCP Summer Drama Camp. I hereby release and hold harmless NGCP, its members, and volunteers from all liabilities and any mishaps that may befall the camper including, but not limited to, the activities in the theater, back rooms, and outside the building. In case of emergency, NGCP has my permission to secure emergency medical treatment for the child listed above.
